Types of Pressure Care Products

Pressure Care Products are essential support aids designed to assist individuals with limited mobility who are at risk of developing pressure sores or who spend extended periods in bed or seated. These products help distribute body weight more evenly, reduce friction and shear, and promote healthy circulation to protect the skin and underlying tissue. This section covers three key categories of Pressure Care Support:

1. Adjustable Beds

Adjustable Beds are highly effective assistive devices, offering a range of benefits — most notably pressure relief, improved comfort, and better body support.
Available in various models and configurations, these beds fall into two primary categories:
1. Hospital Adjustable Beds
2. Home Care Adjustable Beds
While both types offer Manual, Semi-Electric, and Fully Electric options to suit different needs and budgets, there are distinct differences between the two, so it’s important that you are clear on both your needs and the differences before you decide which one to purchase.

Not all adjustable beds offer the same features, but most include some form of tilt function. This key feature helps improve spinal alignment, promotes even weight distribution, and supports the body in a more natural, restful sleeping position. In addition to pressure relief, tilt functions can offer a number of other advantages, particularly for individuals who:

  • Need to be turned or pulled back in bed.
  • Need to sit up slightly in bed, because they have a chest problem, or want to eat, read, or enjoy other hobbies in bed.
  • Have poor circulation, varicose veins, sleep apnea, snoring problems, back pain, fibromyalgia, leg swelling, etc.
  • Need to transfer from the bed to the chair or visa versa.

1. Hospital Adjustable Beds

Pressure Care Mattress on Hospital Bed

Hospital Beds provide essential support for individuals with limited mobility, those recovering from surgery, or anyone who requires assistance with bed bathing, turning, pressure relief, or getting in and out of bed safely. These beds are designed to adjust at the head, feet, and overall height, making daily care and transfers easier for both patients and caregivers.

A key benefit is that some medical aid providers may cover the cost of purchasing a hospital bed. However, it’s worth noting that hospital beds are typically designed for single-person use, with limited variety in size and style, and they often resemble those found in clinical settings.

There are three main types of adjustable hospital beds:
– A Manual Adjustable Hospital Bed
– A Semi Electric Adjustable Hospital Bed
– A Full Electric Adjustable Hospital Bed

Each type offers varying levels of convenience, control, and cost — which we explore in the sections below.

1) Manual Hospital Beds
Manual Hospital Beds

Manual Hospital Beds are adjusted using hand cranks to raise or lower the head, foot, and overall height of the bed. They offer a cost-effective solution, particularly for patients who either have a caregiver or are physically able to operate the cranks themselves. Because all adjustments are done manually, these beds do require physical effort to change positions. If the patient does not require frequent repositioning, a manual bed can be a practical and affordable choice.

While manual beds typically offer fewer positioning options than electric models, they remain a reliable and economical option when basic functionality is all that’s needed — especially when a caregiver is available to assist with adjustments. These Manual Hospital Beds are widely available and come in various models including:

Available for Rent from: Pro MobilityMobility Aids or Solutions Medical or visit our Equipment To Hire Page – Hospital Beds

2) Semi Electric Hospital Beds

Semi Electric Hospital Beds have a motor that controls the adjustments needed for the feet and head, so with the push of a button you can easily change those positions, however the height of the bed is still manually adjusted, so if the patient needs the head and leg rests to be re-positioned frequently, but you do not need the height of the bed adjusted often, a semi electric bed might be perfect for you.

These Semi Electric Hospital Beds are usually a bit more expensive than the manual beds, but you get the added benefit of not having any manual work needed to adjust the two ends of the bed.

3. Full Electric Hospital Beds

Full Electric Hospital Beds are powered through a standard wall outlet and operated using a remote control, typically attached to the side of the bed. This remote allows for easy adjustment of the bed’s various positions — head, foot, and overall height — with no manual effort required.

One of the key advantages of a full electric bed is the ability to raise or lower the entire bed at the push of a button. This is particularly useful when patients need the bed lowered for safer entry and exit, and then raised again to assist caregivers with tasks such as bed bathing or repositioning.

If regular position changes or height adjustments will be necessary, a full electric hospital bed is often the most practical and convenient of the three basic hospital bed types. Below are a few examples of these products available in South Africa:

There is a wide range of manual and electric adjustable beds available in South Africa, each with different features and configurations.
The advantage of Home Care Adjustable Beds is:
– More space for individuals who experience severe leg spasms at night, reducing the risk of limbs falling off the bed.
– Easier turning and repositioning by caregivers using a draw sheet, thanks to the additional room
– Greater flexibility to choose a bed that suits both personal comfort needs and the layout and design of the bedroom.

Below are a few examples of each. Choosing the right one depends on your mobility needs, sleeping habits, and home environment. These products are all available from specialist companies such as Mobility AidsSitwellPro Mobility and Solutions Medical.

Lifting poles and bed rope ladders are assistive devices designed to help individuals with limited mobility reposition themselves in bed more easily and with greater independence. These aids are particularly helpful for people who have difficulty sitting up, shifting their body, or changing position during the night or when waking.

A lifting pole, often mounted to the bed or placed freestanding, allows the user to grip a handle or triangle to lift and adjust their upper body. A bed rope ladder attaches to the foot of the bed and offers a series of handholds that can be used to gradually pull oneself upright. Both devices are simple, low-tech solutions that can greatly enhance comfort, autonomy, and dignity for users at home or in care settings.

3. Furniture Raisers

Bed, table, and chair raisers are simple but highly effective assistive devices designed to elevate furniture to a more practical and accessible height. This adjustment is especially helpful for individuals with mobility impairments, reduced strength, or those recovering from surgery, who may struggle with low seating or transferring in and out of beds and chairs.

Raising the height of a bed, chair, or table can make everyday movements safer and more manageable. It can reduce strain on joints, assist with posture, and provide the necessary clearance for using other essential equipment such as overbed tables, hoists, or mobility aids.

These raisers come in a variety of styles, sizes, and materials, with many being adjustable or modular to fit different types of furniture and user needs.

3. Pressure Relief Products

Pressure Care Products are specially designed to prevent and manage pressure sores (also known as pressure ulcers) in individuals with limited mobility or those who spend extended periods sitting or lying down.

These products—ranging from pressure-relief mattresses and cushions to support pillows—help distribute body weight evenly, reduce pressure on vulnerable areas, and promote healthy circulation. Whether used at home, in hospital, or in a wheelchair, the right pressure care solution can significantly improve comfort, reduce health risks, and support long-term skin integrity and well-being.

1. Pressure Care Mattresses

Pressure Care Mattresses, also known as Pressure Relief Mattresses, are specially designed to help prevent the development of pressure ulcers (bedsores) in individuals who are confined to bed for extended periods due to limited mobility, illness, or injury.

When a person remains in one position for too long, the constant contact between the body and the mattress can restrict blood flow to certain areas. This lack of circulation reduces oxygen and nutrient delivery to the skin and underlying tissue, resulting in painful and potentially serious pressure sores.

To reduce this risk, patients should be repositioned regularly—and ideally, a Pressure Care Mattress should be used. These mattresses are designed to relieve and redistribute pressure, supporting healthier blood flow and reducing the risk of tissue damage.

There are three types of pressure care mattress solutions available on the market today, each offering a different method of protection:
1. Alternating Pressure Mattresses,
2. Foam and Vacuum-Stabilising Mattresses, and
3. Medical Sheepskins
Each play a different but vital role in pressure sore prevention and overall comfort.

Pressure Care Mattress on Hospital Bed
1) Alternating Pressure Mattresses

Alternating Pressure Mattresses are designed to help prevent pressure sores by redistributing pressure across the body. They work by using a series of air cells that gently inflate and deflate at programmed intervals, with the help of an electric pump. This alternating motion promotes healthy blood flow and maintains oxygen supply to the skin and underlying tissues, reducing the risk of skin breakdown.

There are two types of Alternating Pressure Mattresses: 1. Bubble Air Mattresses and 2. Ripple Air Mattresses, each offering varying levels of support and comfort depending on the user’s needs.

Bubble Air Mattress

A Bubble Air Mattress is a type of alternating pressure mattress made up of interconnected bubble-like air pockets. Designed to reduce the risk of pressure sores in individuals who spend long periods of time lying in bed. Typically made from soft PVC or medical-grade materials, it works with a small electric pump that continuously inflates and deflates the bubbles in a wave-like pattern relieving pressure from different parts of the body at intervals.
Bubble Air Mattresses are ideal for persons with limited mobility who spend many hours lying down (e.g. post-surgical patients or elderly individuals) and are at low to moderate risk of developing pressure sores.

Bubble Mattress Overlay

The bubble air mattress is our very cost effective anti bedsore air mattress system. This air mattress system helps with the prevention, management and treatment of bedsores. The mattress system is easy to clean, install and maintain and comes with a two phase silent pump and pvc cover. The mattress design features bubbles both on top and bottom, and have size dimensions of 70 cm (width) x 130 cm (length).

Ripple Air Mattress
Ripple Air Mattress

Ripple Air Mattresses are a type of alternating pressure mattress system designed to provide continuous pressure redistribution for individuals who are at moderate to high risk of developing pressure sores. These mattresses are made up of a series of air-filled horizontal tubes or cells that inflate and deflate in a ripple-like motion, helping to relieve pressure on vulnerable areas of the body, promote healthy blood flow, and reduce the risk of skin breakdown.

They are particularly suited for long-term bedridden patients, individuals with limited mobility, or those with existing Stage 1 or 2 pressure ulcers, and are widely used in hospitals, care homes, and home care settings. The ripple effect ensures consistent movement, comfort, and therapeutic pressure relief throughout the day and night.

The AM 1003 is an effective alternating overlay system combined with static therapy mode.

By having 17pcs of 12.8cm individual cells inflate and deflate alternatively in cycles patients receive adequate pressure relief. The system can be switched to a static mode to facilitate all kinds of nursing procedures, including dressing changes.

With air pressure dial adjustment, the AM 1003 mattress is easy to use and is recommended for patients at medium to high risk of pressure ulcers up to stage three.

The standard class Ripple mattress overlay Saniflow ii already starts with a large-cell mattress overlay system which provides the patient with pleasant relief using alternating pressure.

The advantages of the Saniflow are:
• Overlay mattress system Easy operation: continuously variable pressure setting according to the patient weight, stable folding metal spreader bar for attachment to the bed, simple filter replacement
• Visual display for reaching the required pressure and Visual alarm in the event of pressure loss (e.g. leak)
• Highly elastic foam strips on the rear side and soft feet for noise insulation
• Cycle time 12 minutes and Alternating two-chamber system for intermittent pressure relief
• 17 cells, can be replaced individually; Cell height 13 cm
• Mattress cover made of nylon/PVC fastened to the mattress overlay at the head and foot ends with press studs, water-impermeable, with fold-over ends for fastening to the foam mattress underlay.

2) Foam and Vacuum Stabilising Mattresses

Foam and vacuum stabilising mattresses are designed to immobilize and stabilize a patient, often after trauma or surgery. While they can provide some pressure relief, their primary function is immobilization, not pressure redistribution.

Foam and vacuum stabilising mattresses are available in a wide range of designs, each offering varying levels of pressure relief, support, and comfort. These mattresses differ in construction, pricing, and the degree of pressure redistribution they provide. Vacuum mattresses specifically use a valve to remove air, creating a rigid, custom-fit support. Choosing the right mattress depends on your individual needs, body weight, risk level for pressure sores, and budget.

Since no two people are alike, it’s important to select a mattress that’s best suited to your body type, medical condition, and daily routine. What works well for one person may not meet the needs of another.

3) Medical Sheepskins
Medical sheepskin

Medical sheepskins are natural pressure care aids that can be placed on mattresses, cushions, or other surfaces to provide added comfort and pressure relief. They are especially beneficial for individuals who spend extended periods sitting or lying down, such as those with limited mobility or recovering from surgery.

The soft wool fibres cushion the body, reduce friction, and absorb moisture, helping to keep the skin dry and cool.
By allowing air to circulate around the body, medical sheepskins help minimise heat build-up and reduce the risk of developing pressure sores.

2. Pressure Relief Cushions

For wheelchair users, especially those who have limited ability to shift their weight independently, pressure care cushions are essential. These specially designed cushions not only help prevent pressure sores but also provide critical support, comfort, and stability throughout the day. In many cases, the right cushion is the most important piece of equipment a person can invest in—supporting posture, reducing fatigue, and enhancing seated balance.

Choosing the right cushion is highly individual and depends on factors such as body type, daily activities, and medical needs. The wrong cushion can lead to serious complications, including painful and costly pressure injuries. That’s why it’s important to work with an Occupational Therapist or a seating specialist to identify the best option for you. There are a variety of pressure care cushions available across South Africa including:

Girl sitting in a wheelchair on a Pressure Care Cushion
1. Foam Cushions

Foam cushions are among the most commonly used wheelchair cushions, offering a simple, affordable, and lightweight solution for everyday seating needs. They range from basic single-density foam to multi-layered and contoured designs that provide enhanced pressure management and better positioning support. Some models also include memory foam for improved comfort and pressure distribution.

A foam cushion may be the right choice if you’re looking for: a low-maintenance option that offers basic to moderate positioning support. However, a foam cushion may not be suitable if you: require high-level pressure relief, need advanced postural support or spend extended periods in a wheelchair without weight shifts.

Foam cushions are available from various outlets across South Africa including: Mobility AidsPro MobilitySitwellSolutions Medical, and WINFAR. They come in a wide variety of types, sizes, and configuration including:

2. Gel Cushions
An example of a gel cushion

Gel cushions are designed to offer superior pressure relief by allowing the user’s weight to immerse into a gel-filled surface, which helps to distribute pressure evenly across the sitting area. This reduces the risk of pressure sores and enhances overall comfort. Many gel cushions also feature a contoured foam base for added postural support and stability, making them ideal for users who need advanced positioning.

A gel cushion offers optimal pressure relief for high-risk pressure areas; and advanced positioning and postural support.

Gel cushions are available in a variety of designs and sizes throughout South Africa, including Mobility AidsPro MobilitySitwellSolutions Medical and WINFAR,
They come in a wide variety of types, sizes, and configuration including:

3. Air Cushions

Air cushions are available in a variety of types and styles in South Africa, including popular options such as the XAir Hybrid, Vicair, and Roho cushions. These cushions are generally filled with air, and their firmness can be adjusted by modifying the internal air pressure.

While air cushions typically offer less stability and positioning support compared to foam or gel cushions, they are exceptional for pressure relief, allowing pressure points to gently “sink” into the surface and reducing the risk of pressure sores.

Some models, like hybrid air cushions, combine air with foam elements to improve overall stability and support. Air cushions commonly come in 2-inch and 4-inch heights—with the 2-inch offering a lower profile and greater stability, and the 4-inch providing maximum pressure relief.

An air cushion may be the best option for you if you need exceptional pressure relief; Adjustable firmness and a Light weight cushion, but it may not be best for you if you need:

  • A low maintenance cushion (air cushions require regular monitoring of air pressure, and can develop leaks)
  • Advanced positioning characteristics
  • Utmost seating surface stability
  • A low cost cushion

These Air Cushions which are available in South Africa and include:

4. Hybrid Cushions

Hybrid pressure cushions combine the best features of multiple cushion technologies—typically blending air, foam, or gel—to provide a balance of pressure relief, stability, and positioning.
These cushions are designed to meet more complex seating needs, offering both comfort and clinical support for individuals at higher risk of pressure injuries. By integrating the pressure-relieving benefits of air or gel with the stability and structure of foam, hybrid cushions deliver a tailored solution that can adapt to a user’s body shape, movement, and posture throughout the day. They’re ideal for users who need advanced support but prefer a cushion that is more stable and easier to manage than traditional air-only designs.

Some wheelchair users may need additional support to remain comfortable and properly positioned while seated. While many wheelchairs come with basic backrests, others require more specialized solutions that offer extra padding, postural support, and pressure relief. For individuals who use a wheelchair full-time or for extended periods, the right backrest can be just as important as the right cushion. A well-chosen backrest helps prevent discomfort, poor posture, spinal conditions like scoliosis, and the development of pressure sores.

Backrests come in a wide range of styles, heights, and support levels to suit individual needs. They can be used on both manual and motorized wheelchairs and vary in price and function. Because every person’s body and support requirements are unique, selecting the correct backrest is best done with guidance from an Occupational Therapist or a specialist in seating and positioning. 5. Tips To Selecting & Maintaining a Cushion and Backrest

1. Start With a Proper Seating Assessment

Before choosing any cushion or backrest, a clinical seating assessment by an OT or seating specialist is essential. They will evaluate:

– Pelvic alignment
– Posture and trunk control
– Pressure areas
– Muscle tone (spasticity, flaccidity, asymmetry)
– Sitting tolerance + lifestyle needs

Why it matters: A cushion or backrest is only effective if it matches the user’s body and daily activities.

Once you have selected the type of cushion that suits you, you should remember some of the following:
A seat cushion’s size should be matched exactly to the wheelchair’s seat size
Remember that a seat cushion’s height adds to the wheelchair’s seat-to-floor height, so a taller cushion will cause you to sit taller, accordingly (which can be an issue fitting under tables and desks)
If there’s a chance that the cushion will be exposed to fluids, an incontinence cover should be used (incontinence covers come included with many cushions)
Inspect the cushion regularly for degraded materials or damaged foam
With air cushions, confirm proper inflation daily
Most cushions have an optimal use life span of two to three years, so for proper performance, replace a cushion when needed.
